Before any training sessions Players (or Parents/Guardian if player is 17yo or under) will be invited to consent to the following policies:

GDPR: Please note that in accordance with data protection and GDPR regulations all information that is provided on this form is securely stored, not distributed outside of CT9 Coaching without the express consent of the player (or Parent/Guardian if under 18yo) and is retained only for safeguarding purposes. “CT9 Coaching” is a subsidiary of AHV Consultants Ltd registered at the ICO. 

Urgent medical Attention: Permission for the player to receive urgent medical attention, including emergency dental, medical or surgical treatment including anaesthetic or blood transfusions as deemed necessary by the medical authorities present

Cancellation policy: We understand that situations may arise when you need to cancel or reschedule your session. To do so, we request you notify us at least 5 calendar days in advance. This allows us to offer your session slot to another player on our waiting list.  Cancellations or rescheduling requests made less than 5 calendar days before your session may be subject to a cancellation fee up to 100%. No-shows without prior notification will be charged at 100% of the session cost. Cancellation or reschedule with more than 5 calendar days will not be charged and a full refund will be provided.  In the event that CT9 Coaching has to cancel or rearrange the session an alternative session slot will be offered and also the option of a full refund if new session slot is not convenient

Photography:  CT9 Coaching may photograph or video sessions or parts of sessions for coaching use in to or outside of the session or may take images/videos for use in displays / social media / publications only. All photos and videos will only be used once consent is obtained.
